Troponin I determination

Troponin I is a specific marker of heart muscle (myocardial) injury, used as an aid in diagnosing myocardial infarction. This protein is one of the components of the troponin complex found in the heart muscle and is particularly sensitive and specific to heart cell damage.

The concentration of Troponin I in the blood begins to rise within 3–6 hours from the onset of symptoms, peaks in approximately 12–24 hours, and remains elevated for up to 7 days. Due to these properties, it is particularly suitable for both confirming acute myocardial infarction and late detection, when other laboratory markers have already normalized.

Although troponin I is very specific to the heart, its increase does not necessarily indicate a myocardial infarction. Troponin concentration may also increase in pulmonary embolism, congestive heart failure, sepsis, renal insufficiency, or after physical trauma.

Test principle: Troponin I is determined by an immunochemical method from venous blood. The test is often performed repeatedly at intervals of 1–3 hours to assess changes in concentration.

Recommendations:

• It is recommended for patients experiencing chest pain, shortness of breath, weakness, or other suspected symptoms of heart damage.
• Useful in differentiating myocardial infarction from other non-cardiac origin symptoms.
• Assigned along with ECG, CK-MB, and clinical assessment.
• Suitable for monitoring heart damage after heart surgeries or trauma.
• Test results must be interpreted together with clinical signs and other test data.
