Celiac disease is an autoimmune disorder triggered by gluten in affected individuals. It can manifest not only with digestive symptoms such as bloating or diarrhea but also with iron deficiency, fatigue, weight changes, or other signs. Symptoms alone are not sufficient for diagnosis – celiac disease is determined by evaluating test results, symptoms, and other important health information.
What blood test is most often started with?
For most patients, the main initial serological test is tissue transglutaminase antibodies IgA (tTG-IgA). Since some people have IgA deficiency, the doctor may also assess total IgA or choose tests based on IgG.
Serological tests for celiac disease should be performed while the person is consuming gluten. If a gluten-free diet is followed for a long period before testing, antibody levels may decrease and the test may produce a false negative result. Therefore, gluten should not be eliminated before diagnostics without discussing it with a doctor.
What does the HLA-DQ2 / DQ8 test mean?
A positive HLA-DQ2 or HLA-DQ8 result does not confirm celiac disease, as these genetic variants are also present in many people who do not have the disease. However, if these variants are not found, celiac disease becomes very unlikely, making genetic testing useful in unclear diagnostic situations.
If serological tests are positive or symptoms persist despite negative results, further examination is planned by the doctor; in some cases, endoscopy and small intestine biopsy are necessary to confirm the diagnosis.
