How to prepare for the navigator's health examination?
When arriving for a health check for the navigator's certificate, bring with you:
- Identity card.
- Driver's license (if health is being re-examined).
- Psychiatrist's (mental health center) certificate. Those who do not know which psychiatric institution they belong to should contact their family doctor to get a psychiatrist's report or consult privately with a psychiatrist working at UAB Rezus.lt.
- Personal health history (outpatient card) or a form No. 027/a* issued by the family doctor.
* For persons who do not have a valid form No. 027/a (about health status) issued by a family doctor, or if the clinic where the patient is registered is not in Šiauliai, as well as those not registered in Lithuania, the mandatory health check is performed on-site (tests, electrocardiogram are performed).
Information about the medical certificate for navigators:
The medical certificate for the navigator's health examination is required for taking driving exams and changing the driver's license. The valid certificate needs to be presented to VĮ "Regitra".
A blood test is required for the issuance of the certificate: determination of the blood group ABO and Rh(D), if it is not recorded in your personal document. We perform blood tests on-site, so you will only need to provide blood and we will do everything right there.
