Information about the medical certificate for a firearm:

To obtain a permit to acquire a firearm, a preventive health check must be carried out in a personal health care institution and a certificate confirmed by a family doctor must be obtained. Later, the person must periodically check their health according to the legal procedure.

How to prepare for a health check for a firearm?

When coming for a health check for a firearm, bring with you.

• Identity card;
• Personal health history (ambulatory card) or form No. 027/a issued by the family doctor for illnesses sustained over the last 10 years*.
• Certificate from a psychiatrist (mental health center). Individuals unaware of which psychiatric institution they belong to, to obtain a psychiatrist's extract, should consult their family doctor or privately consult with a psychiatrist working at UAB Rezus.lt.

* For individuals who do not possess a valid form No. 027/a issued by a family doctor (about health status), or if the clinic where the patient is registered is not in Šiauliai city, or those not registered in Lithuania, the mandatory health check is performed on the spot (tests, electrocardiogram are conducted).
