Description

The determination of copper in daily urine is a laboratory test designed to assess the excretion of copper in urine over 24 hours. Copper is a vital trace element involved in many bodily processes, including energy production, iron metabolism, nervous system function, and connective tissue formation. Although the body requires small amounts of copper, excess or deficiency can lead to serious health disorders. Daily urine testing allows for a more accurate assessment of copper metabolism in the body and helps diagnose certain diseases related to the accumulation or elimination of this trace element.

Purpose of the test

The copper in daily urine test is most commonly used to diagnose and monitor Wilson's disease, a rare genetic disorder in which an excess amount of copper accumulates in the body. The test may also be prescribed to assess unexplained liver, neurological, or metabolic disorders. Increased copper excretion in urine can help evaluate the effectiveness of treatment in patients undergoing copper removal therapy. Furthermore, the test can be performed alongside other laboratory tests to provide a more comprehensive assessment of the body's trace element balance.

Symptoms and indications

A doctor may recommend a copper in daily urine test if a patient exhibits symptoms that may indicate disturbed copper metabolism. Common signs include unexplained liver dysfunction, chronic fatigue, tremors, coordination disorders, speech or swallowing difficulties, mood changes, and behavioral disorders. The test is often prescribed when Wilson's disease is suspected, especially if there have been cases of the disease in the family. It may also be used to monitor patients who have already been diagnosed with the disease and are undergoing treatment.

Test procedure

The copper in daily urine test is conducted by collecting all urine for 24 hours in a special container. It is important that the patient accurately follows instructions and collects all urine over the 24-hour period. In some cases, the doctor may recommend avoiding certain dietary supplements or medications prior to the test if they may affect the results. The collected sample is submitted to the laboratory, where the total amount of copper excreted in the urine is determined.

Instructions for proper 24-hour urine collection:

  1. It is best to perform the 24-hour urine collection procedure when you are at home for the entire day.
  2. Keep the urine collection container in a dark and cool place (protected from direct sunlight) for the next 24 hours.
  3. Upon waking up in the morning, discard the first portion of urine into the toilet. Collect the next portion of urine in the container, which will be the starting point for the 24-hour urine collection.
  4. After 24 hours from the starting time, finish the urine collection process into the urine collection container(s).
  5. Mix the total amount of collected daily urine and pour 50 ml into a separate sterile container, labeling it with the patient's name, surname, date, start and end time of urine collection (hour and minute), volume of collected daily urine (ml), and deliver it to the laboratory.

Interpretation of results

Increased copper levels in daily urine are most commonly associated with Wilson's disease but can also be found in certain liver diseases or copper poisoning. Decreased copper excretion usually has little diagnostic significance but may be related to copper deficiency or certain nutritional disorders. The test results are always evaluated alongside other laboratory indicators, such as ceruloplasmin concentration in the blood, copper levels in the blood, and clinical symptoms. If there are deviations from the norm, the doctor may recommend additional tests.

Health significance

The determination of copper in daily urine is an important test for the early diagnosis of copper metabolism disorders and monitoring of treatment. Timely diagnosis of Wilson's disease allows for the initiation of treatment before irreversible liver or nervous system damage occurs. Regular monitoring of copper excretion helps assess treatment effectiveness and reduce the risk of complications. Proper control of copper metabolism is crucial for maintaining good liver, nervous system, and overall body function and quality of life.
