Description

Determination of catecholamines (adrenaline, noradrenaline, dopamine) in daily urine is a specialized laboratory test designed to evaluate the amount of catecholamines - important hormones and neurotransmitters responsible for the body's 'fight or flight' response. This test is performed by collecting urine over a full day and provides valuable information about a person's health status. Catecholamines are crucial for blood pressure regulation, heart rate, and the nervous system's response to stress. Abnormalities in catecholamine levels may be associated with various health issues, making this test important for assessing endocrine and nervous systems function.

Purpose of the Test

The catecholamines in daily urine test helps identify or assess endocrine system and adrenal gland disorders. This test is most commonly used to diagnose rare hormone-secreting tumors, such as pheochromocytoma (adrenal medullary tumor) or paraganglioma, which cause dangerous blood pressure spikes. Additionally, the test can be used to monitor treatment effectiveness in patients who have already been diagnosed with or had these tumors removed.

Symptoms and Indications

The catecholamines in daily urine test is recommended when a patient experiences symptoms that may indicate an excess of these hormones. Common symptoms may include difficulty controlling episodic high blood pressure, strong and frequent heart palpitations, unexplained excessive sweating, severe headaches, anxiety, or tremors. This test is also prescribed if a mass is detected in the adrenal glands during incidental imaging tests.

Test Procedure

The catecholamines in daily urine test involves collecting all urine over a 24-hour period into a special container with a preservative. It is important for the patient to follow instructions precisely and collect all urine over 24 hours. The test processing time after the sample reaches the laboratory is 21 business days.

Instructions for properly collecting daily urine:

The daily urine collection procedure is best performed when you can stay at home for the entire day.

IMPORTANT NOTE: Before starting the daily urine collection, it is necessary to pour the preservative (30 ml of 6N hydrochloric acid) into the urine container. This preservative must be obtained from the laboratory in advance.

  1. Keep the urine collection container with preservative in a cool and dark place (+2...+8 °C, protected from direct sunlight) throughout the process.
  2. On waking up in the morning, discard the first urine portion into the toilet (do not collect it).
  3. All subsequent urine portions must be collected into the same sterile container with preservative.
  4. The morning urine of the next day (at the same time as you started the previous day) is collected into the same daily urine container - this completes the 24-hour cycle.
  5. It is ESSENTIAL to accurately measure the total volume of urine collected over the day and record the total volume in milliliters (ml).
  6. Thoroughly mix the daily urine container, pour 200 ml into a sterile vial, and deliver it to the laboratory. The total recorded daily urine volume (ml) must be clearly indicated on the vial.

Interpretation of Results

Increased levels of catecholamines (adrenaline, noradrenaline, dopamine) in urine may indicate pheochromocytoma, neuroblastoma, or other neuroendocrine tumors. However, it should be noted that temporary increases in levels can be caused by acute stress, intense physical activity, certain foods (e.g., caffeine, bananas, chocolate), or specific medications. If the test results deviate from the norm, an urgent consultation with an endocrinologist is necessary for further examination.

Significance for Health

The determination of catecholamines in daily urine is critically important for the timely diagnosis of potentially life-threatening conditions associated with hormone-secreting tumors. Early diagnosis can prevent severe cardiovascular complications, such as stroke, myocardial infarction, or severe heart rhythm disorders arising from hypertensive crises. Proper determination of these hormone levels helps select precise surgical or medicinal treatment and restore a safe life rhythm for the patient.
