Description

The determination of catecholamines (adrenaline, noradrenaline, dopamine) in 24-hour urine is a specialized laboratory test designed to evaluate the levels of catecholamines - important hormones and neurotransmitters responsible for the body's "fight or flight" response - in the human body. This test is performed by collecting urine over a 24-hour period and provides valuable information about a person's health condition. Catecholamines are crucial for blood pressure regulation, heart rhythm, and nervous system response to stress. Disorders in catecholamine levels can be associated with various health problems, making this test important for assessing the function of the endocrine and nervous systems.

Purpose of the Test

The catecholamines 24-hour urine test helps to identify or evaluate disorders of the endocrine system and adrenal glands. This test is most commonly used to diagnose rare hormone-secreting tumors, such as pheochromocytoma (an adrenal medulla tumor) or paraganglioma, which cause dangerous spikes in blood pressure. Additionally, the test can be used to monitor the effectiveness of treatment in patients with these tumors already diagnosed or removed.

Symptoms and Indications

The catecholamines 24-hour urine test is recommended when a patient experiences symptoms that may indicate an excess of these hormones. Common symptoms may include difficult-to-control, episodic high blood pressure, strong and frequent heart palpitations, unexplained excessive sweating, severe headaches, anxiety, or tremors. This test is also ordered if an incidental imaging test reveals a mass in the adrenal glands.

Test Procedure

The catecholamines 24-hour urine test is performed by collecting all urine over a 24-hour period in a special container with a preservative. It is important that the patient follows the instructions precisely and collects all urine over 24 hours. The test completion time after the sample reaches the laboratory is 21 working days.

Instructions for Proper 24-hour Urine Collection:

The 24-hour urine collection procedure is best performed when you are at home for the entire day.

IMPORTANT NOTE: Before starting the 24-hour urine collection, it is necessary to add a preservative (30 ml of 6N hydrochloric acid) to the urine container. This preservative must be obtained from the laboratory in advance.

  1. Keep the urine collection container with the preservative in a cool and dark place throughout the process (+2...+8 °C, protected from direct sunlight).

  2. Upon waking, discard the first urine portion into the toilet (do not collect it).

  3. All subsequent urine portions must be collected into the same sterile container with the preservative.

  4. The next morning's urine (at the same time as the previous day) is collected into the same 24-hour urine container - completing the 24-hour cycle.

  5. It is ESSENTIAL to accurately measure the total amount of urine collected over 24 hours and record the total volume in milliliters (ml).

  6. Mix the 24-hour urine container well, decant 200 ml into a sterile container, and deliver it to the laboratory. Clearly indicate the recorded total 24-hour urine volume (ml) on the container.

Interpretation of Results

Increased levels of catecholamines (adrenaline, noradrenaline, dopamine) in urine may indicate pheochromocytoma, neuroblastoma, or other neuroendocrine tumors. However, it should be noted that temporary increases in indicators can be caused by acute stress, intense physical exertion, certain foods (e.g., caffeine, bananas, chocolate), or the use of specific medications. If test results deviate from the norm, a prompt consultation with an endocrinologist is necessary for further examination.

Significance for Health

The determination of catecholamines in 24-hour urine is critically important for the timely diagnosis of potentially life-threatening conditions associated with hormone-secreting tumors. Early diagnosis helps to prevent severe cardiovascular complications, such as stroke, myocardial infarction, or severe heart rhythm disorders, that occur due to hypertensive crises. Accurate determination of these hormone levels aids in selecting precise surgical or pharmacological treatment and restoring a safe lifestyle for the patient.
