047/a and 048/a Medical Certificate for Work (preventive health check)

If there are additional risk factors specified in the health check regulations by the Minister of Health Order V-301, additional tests (electrocardiogram, lung X-ray, blood/urine tests, referrals for specialist consultations) may be prescribed by the doctor and will be charged according to Rezus.lt rates.

How to prepare for a health check for work?

When coming for a health check for a work certificate, please bring:

1. Identification card or passport.
2. Employer's completed and signed personal medical booklet (Health Passport No. 048/a) or mandatory health check medical certificate form (No. 047/a), specifying workplace risk factors. The employer can also provide the corresponding electronic form (E048 or E047) through the ESPBI IS system.

Is it necessary to have a lung X-ray?

The doctor evaluates the employee's health and working conditions and decides which tests are needed in each case. A lung X-ray may be prescribed due to certain risk factors, clinical indications, or legal requirements. Depending on the employee's health status and workplace risk, blood, urine, vision, hearing, respiratory function tests, or specialist consultations may also be prescribed.
The final decision on necessary tests is made by the doctor performing the health check, based on medical indications and legal requirements.
In Šiauliai, an X-ray can be performed at the clinic located at Gumbinės g. 33C and in Vilnius at the Rezus Medical Center, Santaros g. 5.

When is it necessary to undergo a preventive health check for work?

A periodic preventive health check is performed for employees before the expiration of the established health certification, following the periodicity set out in legal acts according to their working conditions and occupational risk factors (once every year, once every two years, etc.). This check is mandatory and is conducted in accordance with the provisions of the Labour Code of the Republic of Lithuania, the Law on Safety and Health at Work, and other applicable legal acts.

Periodic preventive health checks of employees are a healthcare procedure during which the employee's health status is assessed to determine if they can safely perform their work functions under current working conditions. Its purpose is to protect the employee's health, prevent occupational diseases and accidents, and ensure the fulfillment of the employer's duties in the field of employee safety and health. After the health check, the doctor records the conclusion and other necessary data in the health check medical certificate or personal medical booklet.

Refusal to undergo a health check, when such a check is mandatory for the employee, is a gross violation of work duties.
