047/a and 048/a Medical Certificate for Work (preventive health check)

*The basic health check price is indicated. If there are additional risk factors as specified in the order of the Minister of Health V-301, regulating the procedure for employee health checks, the doctor may decide to assign additional tests, which are paid according to the Rezus.lt rates.

How to prepare for a work health check?

When coming for a health check for a work certificate, bring with you:

1. Personal identity card.
2. Personal health history (ambulatory card) or a form No. 027/a issued by a family doctor. For individuals who do not have a valid form No. 027/a (about health condition) issued by a family doctor, or if the clinic where the patient is registered is not in Šiauliai city, also those not registered in Lithuania, the mandatory health check is performed on the spot (tests, electrocardiogram are performed).
3. Employer-filled and signed personal medical book (Health Passport No. 048/a) or mandatory health check medical certificate form (No. 047/a), indicating the workplace risk factors. The employer can also provide the corresponding electronic form (E048 or E047) through the ESPBI IS system.

Is it necessary to have a lung X-ray?

If a lung X-ray is required for issuing the certificate (done no later than 1 year ago), a referral for a lung X-ray is issued by UAB Rezus.lt family doctors. In Šiauliai, the X-ray can be performed at the clinic, located at Gumbinės g. 33C. The cost of an X-ray is 15.00 Eur.

When is a preventive health check required for work?

• A health certificate for work is required for employees, unless the doctor specifies another health check date, the employee's health (in the presence of factors indicated in the table) is checked once every 2 years.
• For employees who are allowed to work only after a prior check-up and later periodic checks for infectious diseases, the employee's health is checked once a year.

Employee health is checked to:

• evaluate whether employees and those seeking employment can perform specific tasks under specific working conditions;
• protect employee health from possible effects of occupational risk factors (injuries, etc.);
• prevent or diagnose occupational diseases as early as possible.

Refusal to undergo a health check, when such a check is mandatory for the employee, is a gross violation of work duties.
