047/a and 048/a Medical Certificate for Work (preventive health check)

*The indicated price is for the basic health check. If there are additional risk factors specified in the health check regulations outlined by the Minister of Health Order V-301, additional tests may be prescribed by the doctor and charged according to Rezus.lt rates.

How to prepare for the health check for work?

When coming for a health check for the work certificate, bring with you:

1. Identity card.
2. Personal health history (outpatient card) or the form No. 027/a issued by the family doctor. For persons who do not have a valid form No. 027/a issued by the family doctor (on health condition), or if the clinic where the patient is registered is not in Šiauliai city, or who are not registered in Lithuania, the mandatory health check is performed on site (tests, electrocardiogram).
3. The employer-filled and signed personal medical book (Health Passport No. 048/a) or the mandatory health check medical certificate form (No. 047/a), which indicates the workplace risk factors. The employer can also submit the relevant electronic form (E048 or E047) via the ESPBI IS system.

Is it necessary to take a lung X-ray?

If a lung X-ray (taken not later than 1 year ago) is required for the issuance of the certificate, the referral for the lung X-ray is issued by UAB Rezus.lt family doctors. In Šiauliai, the X-ray can be performed by visiting the clinic at Gumbinės g. 33C. The price of the X-ray is 20.00 EUR.

When is it necessary to perform a preventive health check for work?

• For employees requiring a health certificate for work, if the doctor has not specified another health check date, the employee's health (if factors listed in the table are present), is checked once every 2 years.
• For employees allowed to work only after prior and subsequent periodic checks for communicable diseases, the employee's health is checked once a year.

Employee health is checked to:

• assess whether individuals entering or employed can perform specific work under specific working conditions;
• protect the health of employees from potential occupational risk factors (injuries, etc.);
• prevent or diagnose occupational diseases as early as possible.

Refusal to undergo a health check when such a check is mandatory for the employee is a gross violation of work duties.
