Description

Determination of cortisol in 24-hour urine is a specialized laboratory test designed to assess the amount of cortisol—the main stress hormone—in the human body. This test is conducted by collecting urine over a 24-hour period and provides valuable information about a person's health status. Cortisol is important for maintaining hormonal balance, energy regulation, and stress response. Imbalances in cortisol levels can be associated with various health problems, making this test important for evaluating the function of the endocrine system.

Purpose of the Test

The determination of cortisol in 24-hour urine helps to identify or assess endocrine system disorders, especially those related to the adrenal glands. This test is often used to diagnose Cushing's syndrome, which manifests as excessive cortisol levels, or Addison's disease, characterized by insufficient cortisol production. Additionally, the test can be used to monitor the effectiveness of treatment in patients already diagnosed with these disorders.

Symptoms and Indications

The determination of cortisol in 24-hour urine is recommended when a patient experiences symptoms that may indicate a cortisol imbalance. Common symptoms may include persistent fatigue, weight changes, high blood pressure, muscle weakness, or even signs of depression. This test may also be ordered if adrenal dysfunction is suspected or in cases of chronic stress. For preventive purposes, the test may be recommended for patients at increased risk of endocrine diseases.

Test Procedure

The determination of cortisol in 24-hour urine is performed by collecting all urine in a special container over a 24-hour period. It is important for the patient to follow instructions precisely and collect all urine over the 24-hour period. No special preparation for the test is needed, but it is advisable to avoid intense physical activity and stress during the test to ensure the most accurate results. The collected urine is delivered to the laboratory, and test results are usually provided within a few days.

Instructions for Proper Collection of 24-hour Urine:

  1. The 24-hour urine collection procedure is best performed when you are at home for the entire day.
  2. Keep the urine collection container in a dark and cool place (protected from direct sunlight) for the next 24 hours.
  3. Upon waking in the morning, discard the first urine portion into the toilet. Collect the next urine portion in the container; this will be the starting point for collecting 24-hour urine.
  4. After 24 hours from the start time, finish the urine collection process into the urine collection container(s).
  5. Mix the total amount of collected 24-hour urine and pour 50 ml into a separate sterile container, labeling it with the patient's name, surname, date, start and end time of urine collection (hour, min.), total volume of collected 24-hour urine (ml), and deliver it to the laboratory.

Interpretation of Results

Increased cortisol levels in urine may indicate Cushing's syndrome or prolonged stress, while decreased levels may signal Addison's disease or other adrenal dysfunctions. It should be noted that results may be affected by certain medications, stress, or physical exercise. If the test results are abnormal, it is recommended to consult with a doctor who can more thoroughly evaluate the situation and, if necessary, order additional tests.

Significance for Health

The determination of cortisol in 24-hour urine is an important test for timely diagnosis and treatment of endocrine disorders. Early diagnosis allows for the prevention of potential complications, such as cardiovascular diseases, metabolic disorders, or immune system problems. Regular performance of this test in patients at higher risk can help monitor disease progression and prevent more serious health issues. Properly managing cortisol levels can improve overall health and quality of life.
