The project aims to address the issue of high morbidity and mortality rates from diseases preventable by preventive measures in Šiauliai City Municipality. The problem is caused by the following key factors: ineffective primary health care, limited and uneven access to specialized assistance for residents in their living areas; health care is insufficiently patient-oriented, resulting in a long path for patients to access necessary health care services. Increasing the accessibility and effectiveness of health care services provided in the territory of Šiauliai City Municipality can, in the long term, lead to a reduction in preventable mortality rates, the main chronic non-communicable diseases, and expenditures on disease consequences, complications, patient care, disability, etc.

Project Goal – To improve the accessibility and quality of services provided by municipal health centers and strengthen the cooperation of personal health care institutions in Šiauliai City Municipality.

Target Group – Patients registered with health centers (hereinafter – HC) participating in the activity and providing family medicine services at health care institutions; family members/relatives of patients; employees of health care institutions participating in HC activities; HC coordinators; case managers and multidisciplinary specialist teams of health care institutions participating in HC activities

Project Activities:

  1. Preparation of a document regulating the coordination processes of health center activities;
  2. Financing of the health center coordinator's activities;
  3. An effective service delivery model improving the quality and accessibility of health care services for patients with one or more chronic non-communicable diseases
  4. Training of multidisciplinary specialist team members to apply effective service delivery models;
  5. Acquisition of devices and applications for monitoring and evaluating patients' health status both ambulatory and remotely;
  6. Financing of case manager activities.

Project Value Budget –  1,440,075.00 EUR, of which – 1,224,063.75 EUR – EU funds, 216,011.25 EUR – co-financing funds.
